(A) depicts the full thickness nasal alar defect on the patient. Nasal mucosa is incised along the lines in (B) and pulled inferiorly into the defect making the mucosal layer of reconstruction. (C) shows both the cartilage graft laid onto the mucosal layer and planning for the rotational skin flap using a template of the defect. With the cartilage graft sutured in place, the skin flap is rotated into the defect, as depicted in (D). Triangular dog ears are excised to help close the donor site of the skin flap. (E) illustrates the skin flap sutured into place with the pedicle at the lateral boarder and the donor site hidden as a nasolabial fold. Finally, (F) shows the finished reconstruction with the pedicle revised a few weeks later.
